Our services include medical weight management, virtual acute-care visits, aesthetic treatments, laser services, skincare, wellness therapies, and more. We believe in straightforward pricing, individualized treatment plans, and helping our patients make informed decisions about their care. Prescription medications are dispensed by appropriately licensed pharmacies selected based on the medication prescribed, applicable law, pharmacy availability, and the patient's treatment plan.

Telehealth Availability

Telehealth services are currently available to patients located in the following states, subject to applicable state laws and provider licensure: Illinois, Florida, Washington, Nebraska, Arizona, Colorado, Maine, Nevada, Oregon, Connecticut, New Mexico, Utah, Minnesota, New Hampshire, Wyoming, Montana, and Iowa. Telehealth availability is subject to applicable state laws, provider licensure, and regulatory requirements. The availability of services may change as licensing and regulatory requirements change. Your provider will review your medical history, current medications, health conditions, treatment goals, and other relevant information to determine whether medical weight-management treatment is appropriate.
Treatment may include lifestyle recommendations and, when clinically appropriate, prescription medication.

Prescription medications are dispensed and shipped according to the policies and procedures of the dispensing pharmacy.

Medication Treatment
Treatment pricing begins at $230/month and varies depending on the medication, treatment plan, and clinical needs of the patient.
Medication pricing includes the services specified for the applicable treatment plan.
Prescription medication is not guaranteed. A prescription will be issued only when the treating clinician determines that it is medically appropriate.
Medication selection and dosing are individualized. Your provider may adjust, continue, reduce, or discontinue treatment based on your medical history, response to treatment, tolerability, and clinical judgment.
Patients transferring from another medical practice may be asked to provide documentation of their current treatment and medication history.
Compounded Medications
When clinically appropriate and permitted by applicable law, Studio 74 PLLC may prescribe compounded medications.
Compounded medications are not FDA-approved and have not undergone FDA review for safety, effectiveness, or quality in the same manner as FDA-approved medications.
A compounded medication will only be prescribed when the treating clinician determines that it is appropriate for the individual patient and when its use is permitted under applicable federal and state requirements.
Compounded medications are dispensed by appropriately licensed pharmacies pursuant to a valid prescription.
Ongoing Care
Patients receiving medication therapy must follow the prescribed treatment plan and medication instructions provided by their healthcare professional.
Medication should be used only as prescribed. Patients should not change their dose, frequency, or method of administration without direction from their healthcare provider.
Treatment may be modified or discontinued when medically appropriate.
Individualized Dosing
Medication dosing is determined by the treating clinician and may vary between patients.
Dose adjustments may be considered based on the patient's medical history, treatment response, tolerability, treatment goals, and applicable prescribing information.
Patients should never increase, decrease, or otherwise change their prescribed dose without direction from their healthcare provider.

Neurotoxin Services

Say goodbye to fine lines. Jeuveau® and DAXXIFY® are prescription neurotoxin treatments used to temporarily improve the appearance of certain facial lines and wrinkles.
The amount of product used varies based on the treatment area, individual anatomy, desired result, and product selected.
Results and duration vary by individual and product. Neurotoxins are gradually taken up by the body and subsequently removed therefore wearing off in 3-5 months depending on dosing and product chosen.

The PicoElim Tattoo Removal Laser Spot Master is a laser that emits a 2000ps pulse duration at the picosecond level, utilizing advanced technology for the treatment of pigmented diseases, skin rejuvenation, and tattoo removal.
The laser may be used to treat a variety of tattoos, including eyeliner and lipliner, as well as selected pigmented lesions and other appropriate skin concerns.

The Details

Initial weight loss and vitamin consultations are $50. We will discuss what treatment is best for your goals. Follow up virtual consultations (only needed for those with extensive questions) and re-establishing consultations are $50. If treatment includes medication, monthly refills are done using the medication refill online form and does not charge a fee and is the required method for medication refills. Emails and texts are not accepted for refills. Patients who have not had an active medication order within the previous 90 days may be required to complete a re-establishment evaluation before additional medication can be prescribed. Compounded Medications When clinically appropriate, Studio 74 PLLC may prescribe compounded medications pursuant to applicable federal and state laws. Compounded medications are not FDA-approved and have not undergone FDA review for safety, effectiveness, or quality. The prescribing provider will determine whether a compounded medication is appropriate based on the individual patient's medical history, clinical needs, and treatment goals. Compounded medications are dispensed by appropriately licensed pharmacies pursuant to a valid prescription. Availability of compounded medications may vary based on applicable laws, pharmacy requirements, and medication availability.

Clinic Policies

Medication Continuity & Refill Policy
To promote safe medication use and appropriate clinical follow-up, patients are expected to follow their prescribed treatment schedule and maintain timely communication with Studio 74. Prescription Refills
Patients receiving medication through Studio 74 must submit refill requests through the designated medication-order system.
A prescription refill is not automatic and is subject to clinical review. Treatment Interruptions
If a patient has been without medication for an extended period, the treating clinician may determine that a dose adjustment, additional evaluation, or re-establishment visit is necessary before treatment is resumed.
The appropriate restarting dose will be determined by the treating clinician based on the medication, length of interruption, prior dose, patient history, and clinical circumstances. Medication Vials
Patients must follow the storage, handling, administration, and expiration instructions provided with their medication.
Medication should not be used beyond the period specified by the dispensing pharmacy or treating clinician. Safe Medication Use

Other Permitted or Required Uses and Disclosures We may also use or disclose your PHI when permitted or required by applicable law, including: As Required by Law We may use or disclose your PHI when required by federal, state, or local law. Public Health Activities We may disclose PHI for public health purposes permitted by law, including certain activities involving disease prevention, public health reporting, or product safety. Health Oversight We may disclose PHI to health oversight agencies for activities authorized by law, including audits, investigations, inspections, and licensure activities. Judicial and Administrative Proceedings We may disclose PHI in response to a court or administrative order, subpoena, discovery request, or other lawful process when permitted by law. Law Enforcement We may disclose PHI for law enforcement purposes when permitted or required by applicable law. Research We may use or disclose PHI for research purposes when applicable legal requirements and privacy protections are satisfied. Organ and Tissue Donation We may disclose PHI to organizations involved in organ, eye, or tissue donation and transplantation when permitted by law. Workers' Compensation We may disclose PHI as authorized by laws relating to workers' compensation or similar programs. Military and Veterans If you are a member of the armed forces, we may disclose PHI as required or permitted by applicable military authorities. Inmates If you are an inmate or otherwise in lawful custody, we may disclose PHI when necessary for purposes permitted by law. 3. Uses and Disclosures Requiring Your Authorization Certain uses and disclosures of PHI may require your written authorization under applicable law. Except as permitted or required by law, Studio 74 will obtain your authorization before using or disclosing PHI for purposes requiring authorization. You may revoke an authorization in writing to the extent permitted by law. Revocation will not affect actions already taken in reliance on the authorization. 4. Your Rights Regarding Your PHI You have the following rights regarding your PHI: Right to Inspect and Copy You have the right to inspect and obtain a copy of certain PHI maintained by Studio 74, subject to applicable legal exceptions. Requests should be submitted in writing to our Privacy Officer. Right to Request an Amendment You may request that we amend PHI that you believe is inaccurate or incomplete. Your request must be submitted in writing and should explain why you believe the information should be amended. Right to an Accounting of Disclosures You may request an accounting of certain disclosures of your PHI made by Studio 74, subject to applicable exceptions. Right to Request Restrictions You may request restrictions on certain uses or disclosures of your PHI. Studio 74 is not required to agree to every requested restriction, except where required by applicable law. Right to Request Confidential Communications You may request that we communicate with you regarding your PHI by a particular method or at a particular location. We will accommodate reasonable requests when required by law. Right to a Paper Copy of This Notice You have the right to receive a paper copy of this Notice, even if you have agreed to receive it electronically. Right to Notification Following a Breach You have the right to receive notification as required by applicable law if a breach of your unsecured PHI occurs. 5. Electronic Transmission of PHI Studio 74 uses appropriate safeguards designed to protect PHI transmitted electronically. Electronic communications and transmissions involving PHI will be handled in accordance with applicable privacy and security requirements. 6. Your Right to File a Complaint If you believe your privacy rights have been violated, you may file a complaint with Studio 74's Privacy Officer. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a privacy complaint. 7. Changes to This Notice Studio 74 reserves the right to change this Notice. Any revised Notice will apply to PHI that we already maintain as well as PHI we receive in the future, to the extent permitted by law. The current Notice will be available on our website and in our office. The effective date will appear on the first page of the Notice. 8.
